Sodium bicarbonate is a solution/tablet that corrects excess acid in the blood (acidosis) and also serves as an occasional antacid and to alkalinise urine in certain poisonings.
Also known as: NaHCO3, Bicarbonato
Sodium bicarbonate raises gastric pH and urinary alkalinity, potentially altering ciprofloxacin absorption and excretion.
Raised gastric pH and Na+ can reduce quinolone solubility/bioavailability; alkaline urine changes excretion (Cipro label: avoid antacids; Prontuário).
Clinical infection response; urinary pH if on forced alkalinisation.
Persistent fever, lack of antibiotic response.
Separate antibiotic dosing from the alkalinising solution; monitor clinical response.

Sodium bicarbonate and calcium must NEVER be given in the same infusion line: calcium carbonate precipitation with embolisation risk.
Direct chemical reaction: Ca2+ + HCO3− → insoluble CaCO3 (precipitate), especially at high pH; documented in the calcium gluconate profile and IV labels.
Check line patency; ECG during electrolyte corrections.
Visible precipitate in the line, embolism (rare), arrhythmias.
Use separate lines with a saline flush between drugs; never Y-mix.

IV protocol/emergency use — no food interaction applicable; casual oral use is obsolete.
No dietary restrictions associated with the IV route.
Prontuário Terapêutico INFARMED (bicarbonate — IV protocol use).
Sodium bicarbonate is alkalinising — used with extreme caution in metabolic acidosis (rebound alkalosis, hypernatraemia and left-shift of the O2 curve).
Reserve for documented pH < 7.1 with blood-gas monitoring.
Site monograph (bicarbonate — monitor potassium, sodium and ionised calcium).
Sodium load — caution in hypertensives and cardiac/renal failure (sodium retention).
Monitor sodium and fluid balance with repeated infusions.
Site monograph (caution in renal, cardiac and hepatic failure — sodium retention).
No restrictive Annex 1 entry; IV sodium bicarbonate is used in neonatal resuscitation and correction of severe maternal acidosis with no fetal risk signal. Prolonged oral antacid use can cause alkalosis and sodium overload — avoid chronic use.
Compatible for acute IV use (resuscitation, severe acidosis); avoid chronic oral use (sodium overload, alkalosis).
Compatible; excretion into milk is clinically irrelevant at acute doses.
Prontuário Terapêutico INFARMED — 2.2 (Acid-base balance correctors).

Plasma and urine alkalinisation; correction of metabolic acidosis with raised pH and serum bicarbonate.
Buffers H+ into CO2 and H2O via carbonic anhydrase; CO2 is eliminated by the lungs (insufficient ventilation → paradoxical intracellular acidosis).
Oral: complete intestinal absorption; IV: extracellular volume distribution.
No hepatic metabolism; renally excreted as bicarbonate.
~ 6–12 h (prolonged urinary alkalinisation).
